One thing I'd be careful about because you have diabetes, fit is more important than simply choosing the widest available size. You don't want your toes touching the front or sides, but you also don't want excessive movement inside the boot that could create rubbing or pressure points.
If you normally wear 10 X-Wide in other Propet shoes, I'd be comfortable starting with 10 X-Wide. If your feet swell, you have a very wide forefoot, or you use a thick/custom diabetic orthotic, the 10 XX-Wide (5E) may be worth considering.
