If your main goal is offloading sesamoiditis, I'd say the PowerStep Pinnacle Plus Insoles are potentially helpful but not ideal as a standalone solution. A standard metatarsal pad sits behind the metatarsal heads. Depending on your foot mechanics, it may: decrease pressure under the sesamoids, do almost nothing, or occasionally increase pressure toward the first ray (big-toe side).
