"Accommodates Prescribed Orthotics" - is that size equals foot with orthotics, or size equals foot alone?

It may sound a bit ridiculous to ask, but I ask because $240 to me is a lot for a pair of shoes (though I'm not a total stranger to expensive shoes), so I'd just like to be sure before ordering. My foot is a 10.5 5E, though with my orthotics worn my shoe is an 11 6E. Should I order the 11 6E to accommodate my orthotics, or do I order the 10.5 5E as the shoe is already accommodating for my orthotics? I hope that's somewhat understandable. Thanks.
